Question about item making thing?

Do you just get to alter the item by enchantmenting it, naming it, and commenting on it only? Or do you get to draw your own skins for it? Let's say making a sword with a blue-ish tint to the mettle and a lions face insignia at the top of the handle. Is it possible?
